Cosán – Maximize Your Medicare Benefits
Working in close partnership with Advanced Urology Centers of New York (AUCNY), Cosán is a Medicare-supported program that provides monthly telephone check-ins and care coordination to help you manage your symptoms and stay on track with your health goals.
If You Have Two or More Chronic Conditions, You May Be Eligible
Cosán partners with healthcare providers across the United States to help improve the health outcomes of Medicare beneficiaries who have been diagnosed with two or more qualifying chronic conditions.
Cosán Benefits
If you qualify for the Cosán program, you may receive the following benefits:
- At least 20 minutes of care management services each month
- Monthly telephone check-ins to help you stay on track and provide ongoing support
- Coordination of care between your:
- Pharmacy
- Specialists
- Testing centers
- Hospitals
- Other healthcare providers
- 24/7 access to your personalized care plan through the Cosán Patient App
- Expert assistance with setting and achieving your health goals
- Personalized support from a dedicated healthcare professional who will help you understand and follow your care plan
Does Medicare Cover the Cost?
These services are covered by Medicare up to 80% and by most other insurance plans.
In many cases, the remaining balance may be covered by a secondary insurance plan (excluding Tricare and Humana).
Please note that Care Management services may still be subject to:
- Your annual insurance deductible
- Coinsurance (typically $12–$15)
- Copay, depending on your insurance plan
